BC Children’s Complex Care Transition Facility

The facility plan for the Children’s Complex Care Transition Centre provides centralized, holistic care and a supportive environment for children with complex health needs and their families.

The Provincial Health Services Authority (PHSA) engaged Inform Planning to develop an integrated clinical service plan and functional program for the redevelopment of their Slocan site. This initiative marks a key step in the project’s capital planning phase, creating a children’s complex care transition facility to support seamless transitions from hospital to home for children and youth with disabilities.

With a focus on continuous innovation, the facility aims to set new standards for high-quality care and ongoing improvements in service delivery.

Key features of the facility include:

  • Family-friendly “care-by-parent” suites for transitional care.
  • Multi-purpose assessment and therapy rooms for diverse needs.
  • Teaching facilities for education and skill-building.

The plan integrates natural light, trauma-informed design elements, and culturally safe spaces to foster comfort and healing. Additionally, the facility may accommodate partner agencies, enabling the effective coordination of services for patients and clients with complex health and social needs.
